HIV Much of what applies to the durable power of attorney for health care also applies to a durable financial power of attorney. One difference between the two is in the responsibilities held by the person with the durable power of attorney. The person with durable financial power of attorney for you can pursue anything to do with business or banking, including signing checks, opening bank accounts, signing promissory notes, selling property, transferring property, signing proxies, or pursuing lawsuits. The durable financial power of attorney can be limited to any one of these jobs or can include all of them. As with the durable power of attorney for health care, the durable financial power of attorney is in effect only if two physicians declare you incompetent—that is, incapable of understanding and communicating. It continues in effect only so long as you remain incompetent.*216\191\2*

Cardio & Blood-Cholesterol Perhaps it seems odd to think that the problem of being overweight is having too much energy, but that is a fact. Stored body fat (adipose tissue) is potential energy. The only way your body will use this stared energy is if you “burn” more energy than you take in. By limiting calorie intake and increasing energy use through activity, your body will have to draw from some of its stored body fat meet your energy needs. If you need to lose weight, a slow and steady approach is safest and more likely to be permanent. One rule of thumb for losing about a pound a week is to reduce 500 calories a day. A combination of cutting back on your calorie intake and increasing the number of calories you “burn” through exercise is an ideal way to achieve a 500 calorie deficit. It is not a good idea to consume fewer than 1,400 calories without your doctor’s advice, because it is d to get the nutrition you need with limited quantities of food. Also, weight loss may be too rapid in some people eating fewer than 1,400 calories Once you are satisfied with weight, your goal will be to ma it by having your calorie intake evenly balance the calories you “burn.’By concentrating more on calorie balance than on pounds preparing yourself at the outset for one aspect of weight loss which is distressing to a lot of people. It is discouraging when you step on the scale after making an honest effort at restricting r calories and increasing activity level, only to find that there has been negligible loss of weight. In reality, this may be a sign that you are doing it right! Indeed, what is occurring is that depots are being used up, but because of exercise you are adding muscle. Weight loss will occur if the negative calorie balance is sustained, but key word will be “consistency.”The need for a sustained, consistent program of negative calorie balance followed by lifelong even-calorie balance means that the program has to mesh with the realities of your daily life. There is little to be gained by a heroic 6-week diet and exercise regimen (even if it is a nutritionally sound one, as opposed to a “crash diet”) if it is so inconvenient that afterward you must eliminate it simply to get on with your life. Rather, apply the same philosophy that works for controlling your cholesterol: incorporate calorie consciousness into your daily life without letting it dominate your life. This approach will give you a head start in ensuring that the dietary principles you learn will become second nature to you.*286\252\8*

Cardio & Blood-Cholesterol It is one thing to observe that people with relatively low cholesterol levels have less chance of getting coronary artery disease, but it is another to conclude that if you lower your cholesterol level you will also lower your risk. What if having a high cholesterol level was just a signal that you are at risk of coronary disease but the cholesterol was not actually the cause of that risk that case, there would be very little point in doing anything to lower your cholesterol level. As a matter of fact, it has taken a great deal of effort and expense to perform studies that address that very point, but the answer now is convincing that there is a role for reducing your cholesterol level. ——Several studies have shown a reduction in the occurrence of coronary disease by one-third to one-half in people who lower their cholesterol levels by diet or medications. In generally if your cholesterol level is high or moderately high and you lower it by 10 percent, you will lower your risk of coronary artery disease by abour~2T) percent. Most of these studies have also indicated that higher levels of HDL cholesterol also confer a lower risk of coronary disease. Additional research trials also indicate that a reduction of coronary disease by lowering cholesterol translates into an actual decline in deaths due to heart disease—a very important bottom line!*245\252\8*

Cancer Pain is usually considered to be one the chief symptoms of cancer, but is not actually a common symptom in most cases of the disease. As many as half the cancers in even an advanced stage, do not cause pain. But where it is present, it may be continuous, gradually increasing in intensity and may become unbearable. Thus, for instance, painless swellings in the breast or in the muscle may indicate an underlying carcinoma or sarcoma, respectively. Lymphomas are usually painless enlargements of lymph nodes or spleen. Space-occupying lesions in the brain will cause symptoms of raised intracranial pressure such as headache, vomiting or visual disturbance.Natural Remedies: The use of garlic and garlic milk has been found highly beneficial in relieving pain. This vegetable should be cut into small pieces and taken with a teaspoon of honey with each meal. Taken over a period of time, it will yield good results in relieving pain and suffering. Garlic milk can be prepared both in cooked and uncooked states. In the raw state, the uncooked form is more powerful. This milk is prepared by adding the pulp of crushed garlic in uncooked buffalo milk. The proportion is four cloves to 110ml. of milk. Another method is to boil the garlic in milk.Hot fomentation can also be applied on the painful part of the body to get relief. *8/355/5*

Healthy bones Osteoporosis Rheumatic Now that you have some grounding in the medical and physical realities of spinal cord injury, what can you expect as a patient in a hospital? How do you deal with medical personnel? What are their roles? As a patient in a hospital, you have entered a new world. It consists of predetermined schedules for visitors, therapies, meals, and waking and sleeping. It has a defined hierarchy of authority. It gives you more (or less) personal care than you probably expect, and minimal privacy.
Schedules and Time ManagementFirst, let’s consider the hospital timetable. Hospitals have their own schedules. You’re awakened early and have your meals early. You usually order your meals from a limited number of offerings. Technicians may come into your room at any time to give you a variety of tests, or you may be removed from your room for tests or therapies. Visiting hours may be set, and at some hospitals, phone service is cut off early in the evening. Just when you get the schedule under your belt, a weekend arrives and the whole itinerary changes! You may find the disruption of some routines and the rigidity of others disorienting during your first weeks in the hospital. You may also experience time as compressed or slowed down, because many tasks will take much longer to accomplish soon after your injury. Yet your situation may seem to change every day, as early recovery unfolds.

Cancer Mother’s milk is considered the perfect food. And, indeed, it is an elixir that bestows untold benefits to the newborn. A lactating mother transfers to her child a temporary immunity to the various diseases that she herself has overcome during her lifetime. It also helps to set up the infant’s permanent immune system. Breast fed babies have fewer allergies. Breast milk acts to speed up intestinal growth. Elements in breast milk also turn on certain genes in the cells of the small intestine, which in response send protein signals to developing immune tissues. Special sugars in human milk feed the growing intestinal flora. These sugars, which are not found in infant formulas, provide food for the beneficial bacteria in the colon. There is no infant formula that can even come close to the miracle of mother’s milk.*13/165/1*

Anti-Psychotics I read one article that says the best thing to do when feeling overwhelmed is to take a “laugh break.” If you’re all worn out and feeling defeated, take time out to laugh. It can actually rejuvenate you.In one psychological study, sixty people were divided into three groups and given equal puzzles to solve. All three groups failed the first set of puzzles. Then the researchers gave Groups One and Two some easier tests, like rearranging scrambled letters into words. Both groups did almost as poorly on this as they had on the first batch of really difficult puzzles. Before taking the same scrambled letter test, however, Group Three was allowed to have a “laugh break.” They got their chuckles out of reading a group of ten cartoons to see which ones were the funniest; then they took the scrambled letter test and scored well. The point is, humor can actually alter your mood. Humor made it easier for Group Three to get motivated. Their laugh break helped them overcome their earlier frustration and do a better job!*23\316\2*

Anti-Infectives In a day in which one sees much of medical practice moving toward protocols and algorithms for diagnosing and treating disease, what make fever of unknown origin (FUO) a “hot topic” is that it does not submit easily to diagnostic and therapeutic protocols. Thanks to advances in radiology, microbiology, serology, and molecular diagnostics, many diseases that were once found only after exhaustive study are now discovered earlier in a fever evaluation.Fever of unknown origin is defined as an illness of greater than 3 weeks’ duration with fevers exceeding 38.1 °C (101°F) on repeated occasions without a diagnosis after 1 week of testing in the hospital. This definition has evolved to allow testing to occur in an outpatient environment. Frequently, an FUO is an atypical presentation of a more common illness, and the clinical experience of a specialist who has seen many presentations of disease can be insightful. Thus, during the evaluation of an FUO, it is reasonable to involve the help of consultants in infectious diseases, hematology and oncology, and rheumatology.*148/348/5*

Skin Care Pregnancy is often considered a time when the complexion blooms, when the skin looks better than ever. Although this is true for many women, there are others who begin to develop skin blemishes at this time. Moreover, women may develop stretch marks, varicose veins and spider leg veins, and suffer from very itchy skin and cracked nipples during pregnancy. There are preventative measures which can help women avoid these problems, and most of them can also be treated once they have occurred.Disfiguring pigmentation, especially of the cheeks and upper lip, commonly occurs in pregnancy and when the oral contraceptive pill is taken. This is caused by a combination of oestrogen hormones and ultraviolet A light, which penetrates windows and most conventional sunscreens. The pigmentation may disappear spontaneously after pregnancy, but in most cases it will lighten during the winter months and darken again in summer.Because ultraviolet A light is not blocked adequately by conventional sunscreens, it is difficult to prevent this pigmentation. It is best to keep out of the sun as much as possible during pregnancy. Outdoors, a sunscreen such as tinted zinc cream, Clinique Continuous Coverage, Dermablend and pancake make-up is the best choices. The newer sunscreens which contain titanium dioxide and parsol 1789 (methoxy-dibenzoyl methane) give good but incomplete protection from ultraviolet A light. These include Ego SunSense, Ego Sun-sensitive, Clinique City Block, Nivea Visage with sunscreen and UV Low Allergenic Formula.Pigmentation may fade after pregnancy if sun exposure is diligently avoided. Otherwise, it is extremely difficult to treat. Fading creams available from chemists are ineffective. However, a prescription cream containing 5% hydroquinone and retinoic acid may help to fade the pigmentation in some women. This must only be used under a doctor’s supervision after pregnancy. The depigmenting lotion does not bleach the skin white, but rather fades it to its normal colour. Initially it produces some redness and peeling until acclimatization occurs. In severe cases chemical peeling may be useful but is not without risks, and must be done by a suitably qualified doctor.
